2 – Use the form below to EMAIL me the TRANSACTION NUMBER (located on the top right area of your PayPal receipt), along with your mailing address, phone number and the name of the person you are “gifting” and any other information you want me to know.
I will send you a Gift Certificate within 5 business days, made out to the gift recipient.
This form is for purchasing a GIFT CERTIFICATE. Please provide the following information in the COMMENT section of this form. 1- The TRANSACTION NUMBER (located on the top right area of your PayPal receipt. 2 - The NAME of the person you want the GIFT CERTIFICATE made out to. 3 - The GIFT CERTIFICATE will be sent to YOU, the PURCHASER, so please list your address in the body of the email. 4 - Please include your phone number in case Dana needs to contact you.
